How to get a MyChart account

To use MyChart, you will need internet access and a private email address. There are three ways to set up your account.

At the clinic

You will get an activation code from a clinic representative. We can help you complete your registration while you’re here.

At home with an activation code

Use the activation code from your care team, plus your date of birth and social seurity number to register from your home computer.

Self sign-up

Enter your name, date of birth and social security number. If the information provided matches your clinic registration exactly, you will be enrolled immediately.

Log in to MyChart

MyChart smartphone app

Complete the sign up steps above and then download the MyChart – By EPIC app for your phone. Once you’ve installed the app, search for “OCHIN” then log in with your MyChart username and password. Most of the tools are available on the app. To see office visit notes or change your password, you will have to log in via a computer.
